1Who you are dealing with

This website is operated by Nova Capital Partners, LLC, doing business as Nova Health and Wellness Center, a wellness and aesthetics clinic at 7425 E. Shea Blvd, Suite 112, Scottsdale, Arizona 85260.

Using this site, booking an appointment or starting a telehealth intake means you accept these terms. If you do not accept them, please do not use the site.

2What Nova is, and what it is not

Nova provides wellness, weight management and aesthetic services in person at its Scottsdale clinic, and physician guided care online through a secure telehealth intake.

Nova is not a pharmacy. Nova does not compound, manufacture or dispense medication. Prescriptions are filled and shipped by licensed pharmacies.

Nothing on this website is medical advice, and reading it does not create a patient relationship. A patient relationship begins when a licensed clinician evaluates you, in the clinic or through a telehealth visit.

3Who provides your care

In-clinic treatments are delivered by the Nova team at the Scottsdale location, under the supervision of Nova’s medical provider.

Care that is prescribed online is provided by Qualiphy PC, an independent telehealth medical group. Its clinicians are physicians, physician assistants and nurse practitioners licensed in the state you are in at the time of your visit. They exercise their own independent clinical judgement.

Nova does not direct, and cannot overrule, a clinical decision made by a treating clinician.

4Where services are available

In-clinic services are available only at the Scottsdale, Arizona location.

Care prescribed online is available through the telehealth provider network in 48 states and Washington DC. It is not available in Alabama or Mississippi.

Your eligibility depends on the state you are physically in at the time of your visit, because a clinician must be licensed there. If you are unsure, call the clinic on 480-307-9130 before you start.

5Prescriptions

Treatment that requires a prescription is prescribed only when a licensed clinician reviews your intake and determines it is medically appropriate for you. Paying for a consultation does not guarantee a prescription, and being declined is a clinical outcome rather than a fault in the service.

Compounded preparations are made by licensed compounding pharmacies. They are not FDA approved products and they are not generic versions of brand name drugs. Brand names are the trademarks of their respective owners.

You must give complete and accurate information about your health, your medications and your history. A clinician can only work from what you tell them, and an incomplete answer can make a treatment unsafe.

6Eligibility

You must be 18 or over to use this site, to book an appointment or to request treatment.

Services are for personal use. You may not obtain treatment or medication on behalf of anyone else, or resell anything supplied to you.

7Appointments and cancellations

Clinic appointments are booked through the booking calendar on this site or by calling 480-307-9130. Please give as much notice as you can if you need to change or cancel, so the time can be offered to someone else.

Nova may need to reschedule an appointment. If that happens you will be contacted directly.

8Pricing and payment

Prices shown on this site are the prices in effect when the page was published and can change. The price of a plan built from your body composition scan is explained in full before anything starts, and there are no hidden fees.

Nova accepts HSA and FSA cards for eligible programmes, and offers Cherry, Sunbit, CareCredit, Affirm and Aftercare. Programme pricing is paid directly rather than billed to insurance.

Financing is provided by third parties on their own terms, and approval is their decision.

9Refunds

Services already provided are not refundable.

If a clinician determines that a prescription treatment is not appropriate for you, you are not charged for the medication.

Once a prescription has been filled it cannot be returned or refunded, because dispensed medication cannot lawfully be resold. If something arrives damaged, tell the clinic within 48 hours on 480-307-9130 so it can be put right.

10Messages from Nova

When you give Nova your phone number or email address, you agree to be contacted about your appointments, your care and your account. You can opt out of marketing messages at any time using the link in the message or by replying STOP to a text. Message and data rates may apply.

Ordinary email and text are not secure medical channels. Please keep clinical detail out of them and use the telehealth platform or the clinic phone line instead.

11Your privacy

How Nova handles your information is set out in the Privacy Policy. Clinical information you give during a telehealth visit is held by the treating medical group under its own obligations, including HIPAA where it applies.

The contact form on this site sends an email to the clinic and stores nothing on the website.

12This website

The content, design, images and marks on this site belong to Nova or its licensors, and may not be copied or reused without permission.

Links to other sites are provided for convenience. Nova does not control them and is not responsible for their content.

Nova aims to keep this site accurate and available, but does not warrant that it will be uninterrupted or error free.

13Limits

Nothing in these terms limits liability for death or personal injury caused by negligence, for fraud, or for anything else that cannot lawfully be limited.

Subject to that, Nova is not liable for indirect or consequential loss arising from use of this website.

Individual results vary. Nothing on this site promises a particular outcome.

14Changes and governing law

These terms may change. The date at the top of this page shows when they were last updated, and continuing to use the site after a change means you accept it.

These terms are governed by the laws of the State of Arizona.
