1What this page covers
This page is about the information Nova holds that is not part of your medical record. That includes things like your name and email address when you contact us, your order history, and the information advertising and analytics tools collect when you browse this site.
Your medical record is different. Protected health information is governed by HIPAA and by our HIPAA Notice, which sets out separate and stronger rights. Nothing on this page reduces those rights.
2Your choices
Stop marketing messages. You can opt out of marketing email and text messages at any time, using the unsubscribe link in any email, by replying STOP to a text, or by telling us directly. You will still receive messages about appointments, prescriptions and orders, because those are not marketing.
Limit sharing for advertising. If you do not want your browsing information shared with advertising or analytics providers, tell us and we will apply that choice to your record. You can also decline non essential cookies in your browser.
See, correct or delete what we hold. You can ask for a copy of the personal information we hold about you, ask us to correct it if it is wrong, or ask us to delete it. Some information has to be kept where the law requires it, and we will tell you if that applies to your request.
3How to make a request
Email info@novahealthandwellnesscenter.com, call 480-307-9130, or write to Nova Health and Wellness Center, 7425 E. Shea Blvd, Suite 112, Scottsdale, AZ 85260.
Tell us which choice you are making and give us enough detail to find your record, normally the name and email address or phone number you used with us. We will acknowledge your request and respond within 45 days. If we need more time we will tell you why before that deadline passes.
We may need to verify who you are before we act, particularly on a request to delete. That is to stop someone else making a request in your name.
4Someone acting for you
You can ask an authorised agent to make a request on your behalf. We will ask for written permission signed by you, and we may still contact you directly to confirm.
5We will not treat you differently
Using any of these choices will not change the care you receive, the price you pay, or how we treat you. If you believe it has, call the clinic and ask to speak to the practice manager.
